HIV PREVENTION
SERVICES The HIV Prevention Division offers a wide range of services
that have been developed or adapted to meet the specific
needs of diverse populations. Our goal is to provide
community, small group and individual level education and
skills building to reduce the risk of HIV among NYC’s most
impacted communities. This includes, but is not
limited to group, individual and community level programming
for black men who have sex with men, women of color, young
people, and men and women of every point along the
trans-spectrum. The Prevention Division also provides
a range of harm-reduction programming onsite and through
mobile units.

HEALTHCARE
The Healthcare Division offers quality medical, dental and
support services to persons living with AIDS and HIV. Our
goal is to increase access to healthcare and assist /support
clients with their efforts to stay engaged in care. This is
accomplished by providing an array of services specifically
designed to meet the needs of persons triply diagnosed with
HIV, chemical dependency and mental illness. Healthcare also
provides medical and dental services to individuals who are
HIV negative. Our staff has expertise in providing care in a
sensitive respectful, compassionate and competent manner.
Our newly remodeled, state of the art care is staffed by
HIV/AIDS specialists, including board-certified physicians,
social services case managers, and a nutritionist. In
addition to medical care, the Center’s HIV/AIDS services
include case management, mental health counseling,
complementary therapies such as Art therapy and nutritional
counseling.

HOUSING
Through our alliance, FROST'D and Harlem United clients will
have access to Housing services that are unmatched anywhere
in New York City. We provide over 400 units of housing for
people living with AIDS including undocumented immigrants,
women and children and persons with mental illness.

Primary medical care is provided by family physicians and
internists at all CHN centers. Referrals for specialty
services are made to hospitals within the communities where
patients reside and with whom CHN maintains agreements. We
emphasize holistic care through providing first-rate medical

Adult Consultative Services is committed to providing
comprehensive and coordinated care for HIV affected adults,
adolescents, and families, regardless of immigration status.
We provide state-of-the-art HIV primary, specialty, and
psychosocial services, including the newest and most

Bellevue Hospital Center HIV Services provides comprehensive
medical care to all people with HIV/AIDS. We have an
interdisciplinary team including primary and specialty

The mission of Community Health
Action of Staten Island
